Patient care before the prescription

A medical weight management program needs an authorized prescriber, an appropriate patient evaluation, and a plan for monitoring and follow-up. In Texas, NP and PA prescribing requires the applicable prescriptive authority and physician delegation within the clinician’s scope.

Our practice helps define clinical responsibilities, review treatment protocols, and maintain physician involvement as care progresses. Medication selection and changes remain individualized clinical decisions.

What the program needs

Prescriptive authority

Authorized NP and PA prescribers need the applicable physician delegation and prescriptive authority agreement, within their licensed scope and the services provided.

Evaluation and monitoring protocols

Clear processes for patient assessment, treatment selection, monitoring, patient education, and responding to concerns or adverse events.

Compounding considerations

Compounded medications are not FDA-approved. Their use and sourcing require review of patient-specific clinical needs and current federal and state requirements.

Clinical support as your program grows

New patients, additional prescribers, and changing medications all affect the supervision your program needs. We work with your clinical team on patient evaluations, follow-up plans, chart review, consultation, and escalation of concerns.

Our service includes on-site visits, hands-on training, skills verification, and a documented supervision work ledger. We review the actual program you operate, including how clinical decisions are communicated and recorded.

What we review with your team

  • Prescriber credentials, scope, delegation, and required agreements
  • Patient evaluation, treatment selection, and follow-up responsibilities
  • Medication sourcing, patient education, and adverse-event response
  • Clinical review capacity as patient volume changes

Our active supervision standard

Our medical practice provides 100% chart review, with at least 10% of all good faith exams personally performed by the active supervising physician. On-site visits, over-the-shoulder training, skills certification and verification, and a documented supervision work ledger support ongoing care.

These are our service commitments, documented in the supervision plan, rather than universal legal minimums. The supervising physician remains responsible for delegated medical acts, and each clinician retains their individual license obligations.

Questions weight loss operators ask us

Can an RN run the program?

An RN can provide nursing care within their scope, competency, and valid orders. An RN license alone does not authorize independent medical diagnosis or prescribing. The program needs an authorized prescriber and a clear plan for monitoring, communication, and escalation; business ownership is a separate legal question.

Do we need a physician if we only do injections?

Medication administration and prescribing are separate responsibilities. A valid clinical evaluation, authorized prescribing, appropriate orders, and the required supervision are still needed when another team member gives the injection.

Can this be run entirely by telehealth?

Telehealth may be appropriate for some visits when the clinical situation and Texas requirements allow it. Our program includes ongoing physician involvement and on-site support. The care plan should identify when an in-person assessment or escalation is necessary.

We already have a physician. Is that enough?

Review the physician’s actual involvement, the scope of the agreements, and the services now offered. Our support includes chart review, consultation, patient evaluations, training, and documented follow-up rather than relying on a physician’s name on an agreement.
